B15: Incident Severities

A list of values that describe all possible severities of incidents as determined by the event, cause and consequences of the incident.

1 Near miss

An incident in which negative consequences were narrowly avoided.
2 Minor

An incident whose consequences were minimal and had minimal effect on all parties.
3 Moderate

An incident resulting in notable consequences causing inconvenience and distress to parties involved.
4 Major

An incident resulting in severe consequences that prevented the vessels involved from completing their planned activities (or that resulted in a fatality).
5 Undefined

The severity of the incident is unknown or is not described by any of the previous definitions.
